Current capacity review

All 92 Indiana counties are within FFAC's service area. Current acceptance depends on the requested schedule and caregiver supply. An existing caregiver may apply when program, relationship, screening, training, and employment rules permit.

Referral steps

  1. Confirm the participant’s waiver, managed care entity or health plan, county, requested service, and authorization status.
  2. Ask whether an existing caregiver is available.
  3. Submit the online referral form or contact FFAC.
  4. FFAC will review program, network, authorization, and caregiver availability before discussing a proposed start plan.
Protect participant information: Do not email or submit Social Security numbers, Medicaid identification numbers, detailed diagnoses, medical records, medication lists, dates of birth, or other unnecessary protected health information.
