Indiana PathWays for Aging Waiver (PathWays)

Generally Indiana Medicaid members age 60 or older who meet program and functional eligibility requirements.

Program participation:UnitedHealthcare: Participating. Humana: Contracted; participation effective September 1, 2026. Anthem participation is not currently confirmed. Services are subject to eligibility, authorization, applicable program or network requirements, and FFAC staffing availability.
