Thinking about changing waiver providers?
Indiana waiver participants generally have provider choice within applicable program and network rules. A participant may ask about FFAC, but the required case-management, care-coordination, plan, network, and authorization process still applies.
A careful transition process
- Contact FFAC with the waiver, health plan, county, authorized service, hours if known, and caregiver situation.
- Ask the case manager, care coordinator, or service coordinator about the applicable provider-change process.
- Confirm FFAC serves the program and is in the applicable network when a network applies.
- Confirm FFAC can evaluate the authorized service and current staffing need.
- Complete caregiver onboarding if you want an existing caregiver to apply.
- Coordinate the authorization and proposed start date before ending current services.
Can you keep your caregiver?
An existing caregiver may apply to work through FFAC. The caregiver must qualify, be hired by FFAC, and complete screening, training, documentation, and employment requirements. Program and relationship rules apply.
What FFAC can and cannot do
FFAC can explain its process, verify current availability, prepare an eligible caregiver, and communicate with authorized parties. FFAC cannot place itself on a service plan, approve eligibility or hours, force an authorization, control another organization’s timeline, or promise a gap-free transition.
