FFAC serves all 92 Indiana counties with Attendant Care and Home & Community Assistance where program, network, authorization, and caregiver requirements are met.

Information we need

• Referring organization and professional contact information
• Participant county, waiver, managed care entity or health plan
• Requested service and approximate authorized weekly hours, if known
• Whether an existing caregiver is available
• Preferred contact method and non-clinical coordination notes

FFAC will acknowledge the inquiry, review current program and network status, discuss caregiver availability or onboarding, and coordinate next steps. Timelines depend on the facts of each referral.
