A clear referral path for professionals
Indiana case managers and service coordinators can refer a participant to FFAC through the professional referral form or by calling 812-521-5022. FFAC reviews current program and network status, requested service, county, authorization information, and caregiver availability without asking for protected health information through the public form.
FFAC serves all 92 Indiana counties with Attendant Care and Home & Community Assistance where program, network, authorization, and caregiver requirements are met.
Information we need
- Referring organization and professional contact information
- Participant county, waiver, managed care entity or health plan
- Requested service and approximate authorized weekly hours, if known
- Whether an existing caregiver is available
- Preferred contact method and non-clinical coordination notes
FFAC will acknowledge the inquiry, review current program and network status, discuss caregiver availability or onboarding, and coordinate next steps. Timelines depend on the facts of each referral.
