Indiana PathWays for Aging Waiver is one of Indiana’s Home- and Community-Based Services (HCBS) waivers. Waivers let eligible people receive authorized supports in community settings as an alternative to institutional care.

Who the program generally serves

Generally Indiana Medicaid members age 60 or older who meet program and functional eligibility requirements.

Eligibility and level of care

Eligibility generally includes Medicaid financial rules and program-specific functional or diagnostic requirements. “Level of care” is the assessed support threshold for the program. FFAC does not perform or approve these determinations.

How services are coordinated

A case manager, service coordinator, health plan, or program-specific coordinator helps develop and maintain the participant-centered plan. Services must appear in the approved plan before FFAC can provide them.

Attendant Care and Home & Community Assistance

Attendant Care

Attendant Care may support authorized personal routines, activities of daily living, mobility, safety, and community participation. The current service definition and approved plan control the tasks and hours.

Home & Community Assistance

Home & Community Assistance may support authorized household routines and community functioning. It is non-skilled support, not medical treatment, and must be included in the approved plan.

Provider choice and an existing caregiver

Participants generally choose among providers available for the program, service, county, authorization, and applicable network. An existing caregiver may apply to work through FFAC, but the person must qualify, be hired, and complete screening, training, documentation, and employment requirements. Relationship rules and authorization still apply.
