Generally Indiana Medicaid members age 60 or older who meet program and functional eligibility requirements.

Does FFAC serve all 92 Indiana counties?

All 92 Indiana counties are within FFAC’s stated service area. A service start still depends on the participant’s program, authorization, county, staffing, and caregiver availability.

What services does FFAC provide?

FFAC provides authorized Attendant Care and Home & Community Assistance, also described as Homemaker Services, in participants’ homes and communities. The applicable waiver and approved person-centered service plan control the authorized activities.

Which Indiana Medicaid waivers or programs does FFAC currently serve?

FFAC currently serves the Health & Wellness Waiver and Traumatic Brain Injury Waiver. For Indiana PathWays for Aging, UnitedHealthcare is participating. Humana: Contracted; participation effective September 1, 2026. Anthem participation is not currently confirmed. Authorization, network, county, and staffing requirements still apply.

Can I keep my current caregiver when changing providers?

An existing caregiver may apply to work through FFAC. Employment depends on waiver rules, the approved service plan, relationship restrictions, screening, training, and FFAC hiring requirements.

Can a family member be paid?

Some family members may qualify in some circumstances. The answer depends on the waiver, service, relationship, service plan, and current Medicaid rules. FFAC cannot guarantee approval.

Does FFAC determine Medicaid or waiver eligibility?

No. Indiana Medicaid and the agencies administering each program determine eligibility. FFAC can help you identify questions to ask your case manager, care manager, or service coordinator.

Does FFAC authorize service hours?

Authorized services and hours are established through the participant-centered service plan and the applicable Medicaid process—not by FFAC.

How do I begin services?

Contact FFAC with the participant’s county, waiver or program, health plan when applicable, requested service, authorization status, and caregiver situation. FFAC will confirm its current program and staffing availability before discussing a proposed start plan.

How can a case manager or care manager make a referral?

Use FFAC’s professional referral form or call 812-521-5022. Submit only the non-clinical coordination details requested by the form and do not include protected health information or sensitive identifiers.

Is service availability guaranteed statewide?

No. Statewide service area does not guarantee immediate availability in every county. Availability depends on program and network participation, authorization, staffing, caregiver availability, and applicable requirements.
