This page is for case managers, care coordinators, Area Agencies on Aging, hospital and discharge teams, community organizations, and managed care staff. FFAC does not ask professionals to steer a participant’s provider choice.

Current availability review
FFAC has statewide service scope, but acceptance depends on the county, program or payer, authorization, requested schedule, caregiver availability, and current capacity. An existing caregiver may apply when program, relationship, screening, training, and employment rules permit.

Use the professional referral pathway for non-clinical intake details. Do not submit diagnoses, medical records, dates of birth, Medicaid identification numbers, or other sensitive information through a public website form.
