How to Change Indiana Medicaid Waiver Providers Without a Service Gap

A participant may explore another provider without criticizing the current one. The safest process is coordinated: gather facts first and avoid ending services before the replacement arrangement is ready.

Confirm the moving parts

  1. Tell FFAC the waiver, health plan, service, county, hours if known, and caregiver situation.
  2. Ask the case manager or service coordinator about the program’s provider-change process.
  3. Confirm FFAC’s network and program participation.
  4. Complete caregiver hiring and training if needed.
  5. Confirm the new authorization and proposed start date.
  6. Only then coordinate the end of current services.

No one can promise a perfect transition

FFAC can communicate and prepare, but it does not control plan processing, another provider, network decisions, or authorization timing. A gap-free transition cannot be guaranteed.
