Humana PathWays services through FFAC

What PathWays is
Indiana PathWays for Aging is a managed long-term services and supports program for qualifying Medicaid members, generally age 60 or older. Humana is one of the managed care entities (MCEs) administering PathWays. The health plan and participant-centered process—not FFAC—determine eligibility, authorization, covered services, and approved hours.
Services within FFAC’s scope
FFAC offers Attendant Care and Home & Community Assistance when the member is otherwise eligible, the service is authorized, FFAC’s participation is effective, and current county and staffing capacity can support the request.
Provider choice and transition planning
A Humana PathWays member may ask whether FFAC is available within the applicable provider-choice process. Do not end current services until participation, authorization, caregiver readiness, and a proposed start date are confirmed.
What to share with FFAC
- County and confirmation that the member’s PathWays MCE is Humana
- Authorized service and approximate schedule, if known
- Existing-caregiver information, if someone wants to apply
- Whether the member is currently receiving services from another provider
