Plain-language overview
Family Supports Waiver is one of Indiana’s Home- and Community-Based Services (HCBS) waivers. Waivers let eligible people receive authorized supports in community settings as an alternative to institutional care.
Who the program generally serves
Children and adults with intellectual or developmental disabilities who meet program requirements.
Eligibility and level of care
Eligibility generally includes Medicaid financial rules and program-specific functional or diagnostic requirements. “Level of care” is the assessed support threshold for the program. FFAC does not perform or approve these determinations.
How to apply
Apply through Indiana’s Bureau of Disabilities Services (BDS). Medicaid and program-specific eligibility are separate requirements.
How services are coordinated
A case manager, service coordinator, health plan, or program-specific coordinator helps develop and maintain the participant-centered plan. Services must appear in the approved plan before FFAC can provide them.
