Apply through Indiana’s Bureau of Disabilities Services (BDS). Medicaid and program-specific eligibility are separate requirements.
How services are coordinated
A case manager, service coordinator, health plan, or program-specific coordinator helps develop and maintain the participant-centered plan. Services must appear in the approved plan before FFAC can provide them.
