Children and adults with intellectual or developmental disabilities who meet program requirements and applicable priority criteria.

Eligibility and level of care

How services are coordinated

A case manager, service coordinator, health plan, or program-specific coordinator helps develop and maintain the participant-centered plan. Services must appear in the approved plan before FFAC can provide them.
