Indiana waiver participants generally choose among providers available for their program, service, location, authorization, and network. Knowing what to ask can help you recognize the right fit on a provider-choice list.

Questions to ask each provider

Availability and fit

  • Do you serve my county and program?
  • Are you in my PathWays plan’s network, if applicable?
  • Do you have staffing for my authorized schedule?
  • Can my existing caregiver apply?

Communication and continuity

  • What happens when my regular caregiver calls off?
  • How are schedule changes communicated?
  • Who handles questions or concerns?
  • How do you work with case management or care coordination?

If you later want to switch

Ask how the provider coordinates a transition and what must be confirmed before current services end. No provider can guarantee that authorization processing or staffing will create a gap-free transition.

How to ask about Family First

If Family First sounds like the right fit, ask your case manager or care coordinator whether Family First Attendant Care is available for your program and service. FFAC cannot place itself on your plan or ask a professional to steer your choice.
